BASIC INFORMATION

FULL NAME: Keep it in style
ALIASES/NICKNAMES: If you're character goes by other names then put them here
GENDER: Male or female?
AGE: How old is your character?

CHARACTER

PERSONALITY: Please give a healthy mix of good and bad traits and make your character realistic!! I don't want any Mary-Sues or Gary-Stus!!
STRENGTHS: Physical strengths and personality strengths (don't abuse this section :P)
WEAKNESSES: Balance out your strengths in this section!
LIKES: From colours to people to places, write it all here!
DISLIKES: Everyone has something that gets on their nerves...
AMBITIONS: What does your character hope to achieve, can be great or humble.
SECRETS: Spice everything up a bit ;)
ACTIVITIES: Sports, hobbies, past-times. You can even add detail as to which activites your character likes/dislikes!

APPEARANCES

PLAY BY: Name a real-life person to play your character. Sorry, no cartoons or anime :(
DESCRIPTION: If you're stuck here's some ideas: hair colour/length/style, eye shape/colour, skin tone, build, height, weight, distinctive features, markings, overall ipression given by appearance...

STATUS

BIRTHPLACE: The realm in which your character was born in
FAMILY: Give a very brief description of your character's relationship with each member and if you want you can add descriptions of each member...
HISTORY: Your character's history makes up part of who they are, so don't pass over this but you don't have to make it too long if you don't want to. And again, don't abuse the abused child/orphan story :P

OTHER

ALLEGIANCE: It can be a house, a group, a person, etc.
RESIDENCE: Where they currently reside, not applicable if they're a wanderer
CLASS: Nobility, Royalty, Small folk, and so on and so forth... usually determined by your parents' status and occupation (but there are exceptions...)
OCCUPATION: What your character does.. or suppose to do
RELIGIOUS BELIEFS: Do they worship the Seven? R'hollor? Death? The Old Gods? Or a completely new religion? Remember, it is highly unlikely for a character not to have religious beliefs.
WEAPONS: Doesn't have to be made of steel, but whatever your character uses...
PETS: don't start rambling off about direwolves, dragons.....
